Client intake forms

The questions you ask before a first session, asked once, stored safely and waiting for you when the call starts.

Why an intake form helps

Without one, the first twenty minutes of a session go to basics: background, goals, what the client has tried, the deadline that brought them to you. A short form handled before the call moves those answers ahead of time, so the session starts on the real work. It also tells you early when someone would be better served by a different kind of help.

What to ask

Keep it short. Every extra question costs you a few people who never finish. We help you cut the list to what you'll read before the call:

PracticeTypical questions
Career coachCurrent role, the change they want, timeline, what has stalled so far
TutorStudent's grade and subject, upcoming tests, how they learn best, parent contact
ConsultantCompany size, the problem in a sentence, who decides, any deadline
Habit or wellness coachGoals, routine, what they want from a coach, preferred check-in method

Where it sits in the booking flow

The form can come before booking, as a short application, or right after, as part of the confirmation. Before works well for high-touch programs where you want to screen fit. After works for tutoring and single sessions, where a quick booking matters more. Many scheduling tools can show intake questions inside the booking step, and we use that when it covers what you need.

Where answers go

Answers can land in your email, a private spreadsheet, your client management tool or the scheduling tool itself. We keep the form to the information you need, explain where it's stored, and avoid asking for anything sensitive that a website form shouldn't hold. If your work calls for stricter handling, we'll say so and point you to a tool built for it.

Questions

Can returning clients skip the form?

Yes. Package clients can get a separate booking link with no intake questions.

Can I change the questions later?

Yes. We set the form up so you can add, remove or reword questions without help.
